Use this form to notify each company that automatically debits your checking for payments. Print one copy for each automatic payment change needed.

Social Security Direct Deposit Sign Up Form
Use this form to transfer your social security checks (if appropriate).

Request to Close Account Form
This is the form you will send to your current financial institution instructing them to close your current checking account.
